How Long Does Breast Reduction Surgery Take?
Posted June 19, 2026 in Breast Reduction

Disproportionately large breasts are more than an aesthetic concern. They can also lead to back and shoulder pain and skin irritation underneath the breasts. Women with excessively large breasts often have difficulty finding bras and clothing that fit and may avoid certain activities due to physical discomfort and embarrassment. Breast reduction (mammaplasty) offers a life-changing improvement in breast size and weight, but how long does the surgery take? This blog covers the duration of breast reduction surgery and the factors that may shorten or lengthen the procedure.

What Is the Average Time for a Breast Reduction Procedure?
Breast reduction surgery may take two to five hours, depending on the complexity of your procedure. This number represents actual operating time and does not include the time required for pre-operative preparation and post-operative recovery. You will receive a more accurate time estimate at your initial consultation, after Dr. Cha assesses your needs and goals. However, even this estimate can fluctuate once you are in the operating room and Dr. Cha gets a better understanding of what your procedure will entail.
Factors Impacting Surgical Time During Mammoplasty Surgery
No two breast reductions are exactly alike, which is why it is challenging to predict exactly how long your surgery might take. Factors that can impact the timing may include:
Surgical Technique Used
Different surgical techniques may be used depending on how much correction you require, and each technique affects the timing for mammaplasty. Dr. Cha performs the following techniques during his breast reduction surgeries:
- Periareolar (Donut) Incision: The incision encircles the areola for minor corrections.
- Lollipop Incision: This technique adds a vertical incision beneath the breast, in addition to the periareolar incision, for further correction.
- Anchor Incision: This technique combines the lollipop incision with a third incision along the breast crease for significant reductions.
Amount of Tissue to Remove
The volume of excess breast tissue to be removed may be the most significant factor affecting surgical time. Women with more tissue require more time because it also requires more shaping and repositioning of the breast to ensure the most natural results. Many patients may also need nipple repositioning with most breast reductions, and more tissue removal may require substantial repositioning that can take longer. If the breasts are asymmetrical, it can also take Dr. Cha longer to determine exactly how much breast tissue to remove from each breast to balance the results.
Combination of Treatments
Breast reduction isn’t always performed as a standalone procedure. Large, pendulous breasts are more likely to droop and sag over time. Many patients require a breast reduction and lift combination to position the breasts higher on the chest wall and create an optimal body contour. If fat is present in the breasts with the excess glandular tissue, Dr. Cha may add liposuction to remove the fat. Liposuction can also extend the duration of the surgery.
Patient’s Unique Anatomy and Needs
Your individual anatomy will also affect how long your breast reduction surgery takes. Some of the variables might include:
- Skin elasticity
- Breast density
- Breast asymmetry
- Degree of sagging (ptosis)
- Underlying health conditions
Dr. Cha will consider your individual anatomy when creating your surgical plan at your initial consultation.
Timeline for Your Breast Reduction Surgery Day
The time you spend in the operating room only constitutes a portion of the entire surgical process. This estimate provides a more accurate timeline of how long you will spend at our surgical center on the day of your procedure:
- Pre-operative steps (30 to 60 minutes)
- Surgery (two to five hours)
- Recovery (one to two hours)
Most patients are at the facility for four to six hours or longer. You will go home the same day as your surgery, so make sure you have a friend or family member to drive you home and remain with you for the first day or two.
You may be provided with compression garments to minimize scarring and promote healing. Dr. Cha will provide you with post-operative instructions on how to care for your incision sites, certain medications to take based on your medical history, and a timeline on how long to avoid lifting heavy objects and physical activity.
